INTRODUCTION: Cancer initiation can occur multiple times in chronically injured tissues subject to field cancerization. However, the origins of cancer are poorly understood because it is difficult to follow premalignant cells arising in native tissues. In the liver, chronic injury drives repeated cycles of cell death, regeneration, inflammation, fibrosis, and ultimately hepatocellular carcinoma (HCC). In this context, many cells acquire cancer- causing mutations, yet only a small fraction of these premalignant cells develop into tumors. A defining feature of the liver is metabolic zonation—the compartmentalization of metabolic functions in hepatocytes located in distinct regions of the liver lobule. …
