INTRODUCTION: Heterozygous familial hypercholesterolemia (HeFH)—defined by high concentrations of circulating low-density lipoprotein (LDL) cholesterol—is one of the most common and severe genetic causes of cardiovascular disease. The LDL receptor (LDLR) protein is the primary mechanism by which LDL is cleared from circulation. LDLR binds to Apolipoprotein B100 (the structural component of LDL) via its ligand-binding domain, wherein the complex is endocytosed and and LDL is degraded. Variants in the gene LDLR account for most HeFH cases, and identification of a pathogenic LDLR variant has diagnostic and therapeutic value: Lifestyle changes and the early adoption of lipid-lowering therapeutics can prevent atherosclerosis and reduce the risk of heart attack. …
